Continuing Healthcare
Continuing Healthcare is package which is for individuals outside of hospital who have ongoing health care needs. You can receive (CHC) in any place, including your own home or in a care home. We understand the requirements from the CHC teams.
Furthermore, We understand the requirements laid out by the Department of Health in the National Framework of NHS Continuing HC and NHS-funded Nursing Care. CHC teams and families can expect from us specialists with understanding and requirements for this group of service users. Also, For fast-track palliative patients, we aim to commence packages the same day we receive referrals.
