Being able to deliver the highest standard of care, you must understand the term palliative and end of life care. We understand the importance for quality end of life care, all our team are trained in palliative, end of life care. However, we have a selected and dedicated team that attends to this group of people. Also, Care for this group of service users is led by the nurse manager who has various nursing achievements including the QELCA course. Our palliative care includes not just the service users but also their family and friends. Sometime it might be just a listening ear or some advice.

Furthermore, this group of service users care is often funded by Continuing Health Care(CHC) Our nurse manager has extensive knowledge and experience around Continuing Healthcare, she has worked as a CHC assessor and has assessed hundreds of cases. We understand the requirements from the CHC teams. We understand the requirements laid out by the Department of Health in the National Framework of NHS Continuing Healthcare and NHS-funded Nursing Care. CHC teams and families can expect from us specialists with understanding and requirements for this group of service users. For fast-track palliative service users, we aim to commence packages the same day we receive referrals.

However, If you are in an acute setting looking to be discharged home, we will give your care package priority in getting you home as quickly as possible. If you are fast tracked via Continuing Health Care, we will pick up your care package immediately. Also, We will formulate your package using the fast track document completed by your multidisciplinary team. Whatever the request is we will endeavour to facilitate the request.
