The Complex Care Service is for patients requiring various types of clinical intervention. The aim of the service is to support patients with complex health needs at home by providing care to optimise the highest quality of life and independence. Baxters Homecare experience in delivering complex care in the home means that our careful approach is ideally suited to delivering care of the highest quality, at all stages of the condition. At Baxters Homecare we understand the importance of you living as independently in your own home as long as you wish. We are very pleased to be able to care for you in your own home even if you need artificial feeding, this can be done via continuous infusion or bolus. Please contact us for more information

Specialist Palliative Care
We understand the importance of end of life care, all our team are trained in palliative, end of life care. However we have a selected and dedicated team that attends to this group of people. If you are in an acute setting looking to be discharged home, we will give your care package priority in getting you home as quickly as possible.If you are fast tracked via Continuing Health Care, we will pick up your care package immediately. We will formulate your package using the fast track document completed by your multidisciplinary team.
We understand that dementia can present some challenges for individuals and their families. Our nurses and carers understand and know all about the stages and types of the disease. We are committed and determined to assist people living with dementia to remain in their own home. We offer a range of packages to meet the need of an individual with dementia. An assessment will be carried out by our specialist dementia nurse, who is a Registered Mental health nurse (RMN).
Continuing Health Care (CHC)
Our nurse manager has extensive knowledge and experience around Continuing Healthcare, she has worked as a CHC assessor and has assessed hundreds of cases. We understand the requirements from the CHC teams. We understand the requirements laid out by the Department of Health in the National Framework of NHS Continuing Healthcare and NHS-funded Nursing Care. CHC teams and families can expect from us specialists with understanding and requirements for this group of service users. For fast-track palliative patients, we aim to commence packages the same day we receive referrals.
Furthermore all of our staff are highly trained, however we have a dedicated team who are trained to exceptional standards to care for this group of service users. What ever the request is we will endeavour to facilitate the request.
